The polycarbophil-carbopol based vaginal gel has a high level of bio-adhesiveness capable of ensuring a prolonged moisturising action on the epithelium of the vaginal mucous membrane2. This gel is capable of adhering to the vaginal mucous membranes thanks to the presence of two characteristic polyacid polymers having a high molecular weight and a molecular structure very similar to that of the vaginal mucous: Polycarbophil and carbopol2. Polycarbophil is hydrophilic and provides direct cellular ‘moisturising’ action resulting from an increased flow of water molecules from outside to the inside of the cells on the surface of vaginal mucous membrane. This action creates a moist film over the vaginal wall hydrating the underlying mucosa3. This movement of water into the cells of the vaginal epithelium is promoted by the particular unique structure of the polymers in this gel2. Being bio-adhesive, Regelle has been shown in laboratory and clinical studies to adhere to tissue for 24-72h, resulting in diffusion of water from the gel to the underlying vaginal mucosa3.

Furthermore, polycarbophil and carbopol are substances with a buffering action and their use in women experiencing menopause either naturally or as a result of surgery has led to a correction of the vaginal pH values comparable to those obtained with hormone replacement therapy4.

The pH of the polycarbophil gel is 3.0 and it buffers to a normal vaginal pH of 3.5 to 5.03.

Sexuality doesn’t stop with age

Due to a lengthening of the average lifespan and longer life expectancy after menopause, vaginal atrophy is destined to become a clinical problem with an increasingly greater epidemiological significance. The seriousness of this menopausal symptom should not be minimized, as vaginal dryness represents an uncomfortable problem, with physical and psychological consequences that can have a deleterious impact on the patient’s quality of life.

It is estimated that up to 40% of your post-menopausal patients may need your advice but are simply too embarrassed to ask. Furthermore, even if you are prescribing HRT to your patients, it is shown that 25% of menopausal women who take hormone replacement therapy continue to present with the signs and symptoms of vaginal atrophy.1

Vaginal atrophy is still very much a taboo subject as many of your patients may too embarrassed to talk to their partners, friends and even doctors about their problems. Due to embarrassment or perceived taboo, patients are reluctant to seek out professional advice and treatment which can have a major impact on your patients everyday vaginal comfort, self-confidence, sex life and ultimately quality of life. Regelle® is a long lasting vaginal moisturiser intended for the symptomatic relief of vaginal atrophy, dryness, itching, irritation and discomfort. 

The recommended Regelle® dosage is as follows:

One application (2.5 g) to be inserted into the vagina every three days, preferably in the morning. 

Maintenance therapy can be achieved with a three times weekly application. Depending on the level of dryness, Regelle® can be used more or less frequently and can be used daily. 

Regelle comes in a pack of 3, 6 or 12 single-use applicators.

Step 1: Grip the applicator by the thick end. Shake down the contents to ensure they’re at the thin end of the applicator.

Step 2:Twist off the tab and discard.

Step 3: The applicator can be inserted into the vagina while you are lying down on your back with your knees bent, or in a sitting position. Gently insert the thin end into the vagina as far as it will go comfortably.

Step 4: Squeeze the thick end of the applicator firmly to deposit the gel. Remove the applicator and discard appropriately.

Clinical Paper 1:
In a study by Gelfand et al (1994) treating vaginal dryness in breast Cancer Patients Polycarbophil Moisturising gel show that a polycarbophil vaginal moisturiser gel effectively relieves vaginal dryness in most patients without causing significant side effects. This study also showed a statistically significant reduction in mean vaginal pH and improvement in overall vaginal elasticity scores, as well as significant improvement in vaginal health measures at each monthly interval.

Clinical Paper 2:
In a comparative study by Nachtigall et al (1994) of a polycarbophil gel versus local oestrogen in menopausal women, a statistically significant return of vaginal moisture and vaginal fluid volume was shown by week 4 in both groups. Vaginal elasticity also increased in both groups. No serious side effects were reported. The result indicated that the bioadhesive vaginal moisturiser was a safe and effective alternative to oestrogen vaginal creams, with both therapies exhibiting statistically significant increases in vaginal moisture, vaginal fluid, and vaginal elasticity with a return of the premenopausal pH state.

Clinical Paper 3:
In a 12 month study by Bachmann et al of peri-menopausal and post-menopausal women, use of polycarbophil gel 3 times per week for one year showed that not only had vaginal health improved but this improvement was maintained as long as subjects were using the vaginal product. All parameter of the vaginal health index showed improvement including vaginal moisture, vaginal fluid volume, vaginal pH, vaginal epithelial integrity and vaginal elasticity. Overall the gel was found be well tolerated and effective in improving overall vaginal health for one year of continuous use. There were no patients who dropped out due to adverse events.

Clinical Paper 4:
In the VALYD study by Mincigrucci et al (2005), the use of a polycarbophil gel, either alone or in combination with HRT, brought about a significant and clinically relevant improvement in the Vaginal Health Index Score and symptoms related to vaginal atrophy. In particular, significant reductions of itching, burning and vaginal discharges were observed Dyspareunia dropped by 84%: this reduction occurred both in women taking a hormonal therapy in conjunction with a non-hormonal vaginal therapy and among women receiving only non-hormonal vaginal therapy.
